Are there any? Pretty much I just mean no final wide release to operators. Prototypes and location tests are OK.
Clockwork of the Aquario could count. It's a game by Westone, creators of Wonder Boy and Monster World. The game never survived past location tests. Last I checked, though, the guy in charge of the company had found a version of it in some computer files and he's trying to get it released in some form.
Slipstream - Capcom 1995 F1 racing game. According to system16.com, only 150 boards were made. Most of them (if not all) were for Brazil and the rest of Latin America. At that time, Romstar Brasil was responsible for distribution and localization of all Capcom arcade games in the region. I only saw this on arcades once. Curiously, this is the only non-Sega game running on System 32 system.
There are quite a few Korean-exclusive shooters & brawlers, but I can't be bothered to make a list right now! Maybe we're lucky and Shou sees this, he's the expert.
There was a website that appeared a while back about some arcade games that were only released in the Soviet Union. I'll try to find it later.
that's immediately what popped into my mind. It had a nice write up attached to it on the subject of soviet game design. For example, their games had no scores, because the idea of competition amongst the people was discouraged. Rather, instead of competing for score, games often offered extra lives or continues as rewards for jobs well done.
I doubt Air Attack was ever released outside Korea. It's interesting because if you played PC games in the 90s, you'll probably recognize a ton of stolen assets. That's obviously Senna in a McLaren in a game released in 1995...

Do you really want to count protos and Korean/Chinese/Taiwanese developed games? This list could go on and on. For actually released games, Taito's Solitary Fighter comes to mind as it is a World only release (meaning export, non-US). It's the sequel to Violence Fight and is incorrectly referred to as Violence Fight II on the internet. Charlie Ninja, Dyna Gear and Gaia Crusaders are all games developed in Japan but never released in Japan or the US. There are also a handful of Japanese developed MVS titles that only came out in Europe like Super Dodgeball and Band Bead.
It's a System 18 game and the data has been sent to Sega for future distribution on their version of the Nesica in Japan along with Fantasy Zone II DX.
You'll find a large proportion of games not released in either the US or Japan will be either localized variants with a different title or bootlegs. This was an Italian clone (I forget the title / what it was a clone of - it was years ago that I saw it): My friend has a cocktail Moon Striker from B.G. Video in the UK, a Moon Cresta clone. IGS PGM was a taiwanese Neo Geo clone you could say but it got unique games with good 2d graphics.Like martial masters or the gladiator arcade
