Looks to be the Final version of Quake 3 on a burnt GD-R. Do you have a System Disc 2 to check the game? Id be interested if your selling PM me.
Yep as "proxxshot" also says, it's a GD-R (these kind of discs were used by developers to burn the game for testing and to ship out for testing to members of the press etc) with most likely the final version (same as the version that got released in stores) of Quake on it.
