    Yes it is the disc only placed in a jewel case. I was not ever in posession of an official insert or case.

    Sorry I forgot to address shipping costs. I have updated the original post accordingly.

    Will provide free shipping within the continental US and will pay half of the shipping to any over seas parties. Shipping would end up at roughly $20 to most locations.
