So, after some research and requesting in this same forum, a good friend shared the FCN release of Metrowerks CodeWarrior for Playstation R5.
This is the latest version of Metrowerks CodeWarrior for PlayStation that was released back in the day (1999)

    The good news:
It seems to be a functional release. I haven't had time to test everything, but the IDE starts up, as well as the PSComUtil.

    The bad news:
The release is not an ISO nor the Setup, but rather an installed copy of the application (ie. they copied an existing installation).
This also means you have to manually create some registry keys.
Additionally, it is incomplete, missing at least the following:
    -Documentation
    -Examples
    -Shortcuts
    -Debugger?
    -Other stuff?

    Additional information:
Based on the installation log, it seems this was originally installed to the default location in a D drive (D:\Program Files\Metrowerks\Codewarrior).
The directory /CW Release Notes/CodeWarrior Tools/Install Notes/ contains the install_log.txt I mentioned, as well as information on File Association and Registry entries; however these have to be manually created.
I tested this in a Windows 7 32-bits VM. The latest patch (5.2) seemed to install ok (ie. no errors); however it left me with no IDE.exe, so either the patch does not really work correctly in Windows 7 (I'll see if I can test later on with older OS), or it kind of fails due to the application not having been installed using the original setup.
In any case, I was able to extract the 5.2 patch and it can be manually "applied".
REQUIRES the official Sony Programmer Tools Runtime Library (a.k.a. PS SDK)!!!

So, below you'll find the following:
    -Original release by FCN
    -Original 5.2 update patch
-FCN release Patched by me, including documentation I found from other CW release (Targeting Playstation OS from R4 update patch, and generic docs from MW CW Pro R3, which should match the IDE version of this release).

You're more than welcome, though thanks should go to FCN for releasing this back in the day, as well as the kind soul that shared it with me.

I wish we had an image of the disc (ISO, BIN/CUE, CCD, etc) or at least the contents of the disc unaltered (ie. in a compressed file). Hopefully someday :)
In the meantime, I'm having a hard time getting some of the generic docs (the ones included with all Codewarrior "editions") from around the date of release of this version; these are not critical, but I wanted to include them. I'll keep looking for them a little longer, and then whether I find these or not, I'll share the manually patched/"completed" version I put together.

'MSL C REFERENCE.HLP' is corrupted. I doubt you can repair it since it was ripped by 'TEAM FCN' back in 1999, but like you said, a 1:1 copy of the original would be nice :)
