SCPH-1002 Console Mod Chip Issue...

Hi all, I've been trying to do my own mod chip install (from scratch for once). I've programmed the PIC using a K150 programmer and using the latest Old Crow mod chip binary from GaryOPA.

I've done it all and put it all together but it's still now loading copies... could anyone with knowledge on this have a look at my work log and see if I have missed something or done something wrong?

Failing that if anyone here has any spare mod chips I would like one... I am thinking perhaps my programmer is too cheap and cheerful...?
