I wanted to know is it normal that when loading a CD using the l_olivaira bios on sega CD (Jap BIOS), does the load screen go to the CD Rom Menu and then you have to select CD ROM to play the game, or should it boot the game straight away

    im a little confused

It's not said anywhere (but on a small line hidden at the manual, in Japanese) that at the screen with clouds on if you push A+B+C and UP it will open the tray. It's great to take stuck discs out ... lol

Holding A is a behavior which was introduced with the US SEGA-CD and PAL MEGA-CD BIOS. And holding the keys you mention on boot will cause their respective menus to come, even if a disc is inserted. SEGA stuff is so interesting, huh ? ;)

A+B+C+UP is for the Japanese BIOS for MEGA-CD with CD TRAY loading (1.0)

It's on the PS2: R1+R2+L1+L2+Select+Start... A lot of game makers follow that combo, but it was not enforced.
On the PS3 and XBOX360 that lost the purpose because these consoles have an sort of sub HUD menu you can bring with a special key, which give you access to special functions such exit a software or re-start it...
