This game itself is semi-rare but hard to find with the doctors sleeve. The entire boxed game fits inside the sleeve and there is a signout list for patients to borrow it from the doctors office on the back.
I actually just recently saw an ebay auction for a complete Captain Novolin, it has box, cardboard insert tray, cart, instructions, poster, and registration card - but I don't think it has this sign-out sheet you have. Here's the auction (well, buy it now only) There's a (crappy) picture.
Its a $20-$40 game without the doctors sleeve. The signout sheet is not a sheet but the back of a larger box which contains the whole shebang. Not that it matters but he is also missing a Nintendo power subscription card.
