I've been searching for about 30 minutes now with my limited knowledge of Japanese, and I've come across some things. Have you seen this one? That's what I thought at first, but look closer; they appear to be 3D models just ripped straight out of the game (or recreated). The scans in the OP show physical dioramas.
I don't think that is the one either, but thanks for the search. In fact, in the same amazon link you sent, if you scroll down you can see user submitted pics of the inside of the guide, and it's clear it's not the one. I think the most telling hint of the outside look of the guide I'm looking for are the side of the pages. Look at the pic attached, you can see how every page has a dark-colored stripe on the right-bottom part. I'm sure there must be a stripe along that spine when the book is closed.
I see what you're saying about the dark stripe. Well, there's this shop near my school that sells retro games (along with a shitload of hentai) and they've got a pretty big shelf of game guides. I'll take a look sometime next week and see if I can find anything.

They used to have models in Japanese guide books quite a lot, along with all the commissioned art... Never seen anything to this extent before, though, pretty amazing.
I wonder if the company who wrote this guidebook also built the actual maps, or were they done by a specialised third party? The copyright/about section of th book should be mentioning this.
Added courses 2 to 8 and the "credits" page. 7 more scans to go. It seems the modelling was mostly outsourced. Notice the studio names in brackets. I really like the dedication and attention to detail. Makes it an unique guide book worth preserving.
