As i just played through Bio Freaks in "Freak" difficulty again *yawns* i wondered if there are more, really difficult Beat 'em Ups. The system or age doesn't matter, i just need something new...
Streets of Rage 2 on Mania. (you need to complete it on Hardest or do the cheat to get mania though) Or Gourmet Sentai Bara Yarou for sfc. An interesting final fight clone with in between round cooking. Some good graphics and oddball enemies make for a fun game.
Some modern ps2, cheap games for you - Street Fighter III is a fighter I always find a bit difficult on the hardest setting. I always wimp out and use my fav characters, try to use twelve, hugo or some of the less popular characters. (although with hugo keep the blocking to a minimum as you can be cheap against a cpu, resist the temptation!) Q is a good character to "learn" as well. Guilty Gear XX is good on the hardest setting as well, although things don't get challenging till the last couple of people in arcade mode. Go with survival mode instead, it gets very serious quite quickly. You get lots of relaxing fights even later on, but when the powered up people turn up things start getting fun. I love doing this mode for as long as possible, though potemkin is often my downfall when he turns up! Play KOF 2001 like you were playing neowave! Go 3 or 4 character teams, no cheap striker combos. So no striking at all. Of course hardest setting. That is the only way cpu KoF can be even slightly challenging. SVC chaos is as hard as how you play. Put it on 8. Use a random character and only use cheap tactics when the bosses turn up. Now melty blood even on the hardest setting is quite easy, but play it anyway. It is just a really solid fighter (my bias creeping in!). Plus you could download the demo within the next 5 minutes! Oh and rumblefish on hardest setting is rock solid from about the fourth fight in. Good luck with that. Maybe I just haven't got into the right frame of mind for this one, but it can be damn hard.

Heh, I honestly thought beat'em ups were the same as fighting games. Seriously, I had never drawn the difference mentally! I always referred to them (beat em ups) as scrolling fighters or something, anyhoo, my mistake. Apologies. Try undercover cops on the SFC
Yeah, you can't beat a Battletoads game for difficulty. Another of my favorite beat'em ups which was really difficult is Captain America & The Avengers. Unfortunately, both the Genesis and the SNES ports were pretty bad, so you'd have to emulate it--or buy the arcade cab, I guess. Most of the Atlus games back to Double Dragon and up through the original line of Kunio games were pretty hard, too (I never played the GBA remakes, so I can't speak to their difficulty). If you want something really hard, try the Atari 2600 port of Double Dragon. Hah!!! ...word is bondage...
Are you credit feeding? Try to do it the way the game was intended -- on one credit... then quit yawning. If you're doing these all on a credit, well, then you got mad kung-fu.

1cc is the only way! Anything else goes against the laws of gaming. Guardian Heroes is on SS and there is another version on the GBA. However, my personal recomendation, perhaps if you enjoy GH is Panzer Bandit on Psone. It can be quite hard as long as you don't abuse some (almost) infinite combos. It is a total rip off of GH but I do prefer the loose and long combos. Well I would place them as virtual equals IMHO. More action in panzer for my money and some serious juggling to boot. The only criticism I would have is that although it has different planes, you don't have to use them at all! Plus some bosses can be abused a la Zenki FX throwing move. Another recomendation would have to be captain commando - I only have played the arcade version. (so can't comment on the psone) but I enjoyed that, I'm sure with the difficulty hiked up it could give you a decent challenge.
I second Battletoads on NES. Scarily enough though, I know a person who beat it on their second try. Nobody believed him, but then he did it for us. Amazing....as the game is just impossible.
Try Captain Commando on the Playstation (or Mame), Dungeons and Dragons Collection on the Saturn (or Mame) -- both should be plenty challenging.
Gameboy Kunio games=hard. I have 2 of them and I got to the second level in one of them and you have to fight 2 twins which as hard as hell to beat plus the game only gave me 1 continue for some reason so I just stopped playing.
