Very cool Maks! Those original CCL I/O adapters are pretty hard to find. I havn't seen that specific "2 IN 1" box before either. I had only seen "2 IN 1" boxes for the PFX Turbo with the Smart Card slot. Could your box be for a PFX bundle which includes the Super Smart Card adapter as well? Until now I was only aware of this box for the PFX: CF
I don't remember bundles ever being sold, the box was probably just more of an advertisement. It has a few pics from the OS with the gameboy file manager on the box.
