You don't actually have to rename it. All MBR programs will check for the fsck folder too.
But...

It was meant to be a 100% compatible, drop-in replacement of the SONY original.

Unfortunately, there seems to be some problems with the fsck module (the module responsible for checking PFS partitions), so it may not be entirely reliable.
The FSCK tool that comes with FHDB is a scaled-down version of the HDDChecker tool's functionality (only the fsck module is included), much like the SCE FSCK tool.

So needlessly to say, for now... it is definitely better to use the SONY FSCK tool.
