This could be outright fake, but still, I wouldnt hold it against Kojima to do something like this. In this pic is what appears to be Solid Snake. The original Solid Snake. PERHAPS this means Old Snake is Big Boss, this Snake is a younger clone, or Solid Snake. This pic is said to have originated from a "hidden" trailer that has been asked to be taken down by Kojima, but its still too cool to not at least talk about.
Nevermind, I think I found the trailer. Takes forever to buffer, though. http://www.konami.jp/gs/kojima_pro/movie/mgs4_tgs2k6.asx
Could be one of 4 things -The younger Snake is a clone of either Solid or Big Boss that we don't know about. (Possible, since the Metal Gear franchise would either need a new Snake or use Ninja Raiden as the new lead). -Old Snake is Big Boss pretending to be Snake, and somehow got his eye back, and the young Snake is Solid(Unlikely) -It is a mask (Pretty much unlikely, since the face twitched, lips moved, and eyes blinked) -It is Solid Snake at a earlier time, like the previous games after MGS2 that have a chapter before the main. Plus, it takes place at different locations and times (Highest chance).
I hope this is the route the story goes. Big Boss could PROBABLY get his eye back from having it transplanted from another clone or something. Also, he probably can stay alive via the drugs mentioned in MGS1 by Naomi that kept the Cyborg Ninja alive past his death. Thats probably what that needle thing is. He may even have life support stuff in his suit, too. Its got camo similar to the Cyborg Ninja suit after all. It could be anything though, because a game that follows a story where the main villain is controlled simply by an arm could do whatever. Besides I think a game set in an era where everyone ELSE hasnt aged at all other than the main character is a little odd.
You would think that would take too long to explain, though...I mean, it does seem a bit odd that the younger Snake didn't use a knife and the older one does all that and stuff Big Boss would do, but they would need to explain: -How he managed to live through Zanzabar -How he foolded everyone into thinking he was Snake -How he got rid of his burns, and how he got his eye back -What his motives are -How he could live to be nearly 80-100 years and still be as fit as he is. I think some of the answers lie in Portable Ops, in my opinion.
I say you begin with young (kinda) snake in a mission somewhere else from the E3 clip, then he gets off the battlefield becos his ageing process goes out of control. Then you resume as old snake, get a lift from otakon and go into the middle east... About old snake being BigBoss, I highly doubt it. First, while he has some scars that could be considered skin implants, you've to remember that in MGS1 they werent talking about burns but that his entire body was burned beyond recognition. Hell, I even remember when they talk about the genome soldiers. They say they had to take the DNA from some hair from BB since the rest of the body was so burned that there was nothing to get a sample of.

It looks nice, but people have illustrated (http://www.the-magicbox.com/forums/showthread.php?t=19047) that the texturing is pretty ropey in parts and a bit rough around the edges. Perhaps this is down to Konami rushing development to get stuff ready for TGS but I'm hoping that will improve. Metal Gear Solid needs that realistic polish.
