    It's possible that an entire batch was backwards, and one found it's way out of that.

    But still, it seems like such an easy thing to hoax that he needs to have more than just some large black caps-lock description of how rare it is for anyone to take him seriously.

    Oh, I bet it's real. This kind of mis-hap happens all the time. I have Hot Wheels cars and action figures that are sealed on their cards upside down (or the card is upside-down/backwards, whatever). If an action figure can get out of the factory packaged upside-down, I'm sure a sticker on a cartridge can easily get out too...

    Sure, it's uncommon, and for some makes a neat collectible, but it's hardly worth much more than the game itself is worth. In some cases less, since it is, technically, screwed up...

    And, in the case of a sticker, it might be easy enough to carefully remove it and flip it (although I suspect you'd see evidence of the crease where it folds over the edge in the case of an NES cart...)
