Master System Black Belt: This was a Fist of the North Star (Hokuto no Ken) game in Japan. They changed Kenshiro into some generic dude in a gi. The generic thugs sprites were also changed as were all the backgrounds. Genesis/Megadrive Last Battle: This was actually HnK 2. The main character sprites had their clothing colors changed and they got rid of the amazingly violent for the time explosions of blood that came out of the thugs when you killed them; here they just fly off the screen. The names and story were changed to protect the incredibly awesome. I wish knowing about Hokuto no Ken games were a marketable skill.

Interesting topic! Only things I can think of right now which have not yet been mentioned: Turok (N64): German version has robots instead of humans and no blood. Shadows of the Empire (N64): I read once that a "very easy" difficulty setting was added to the japanese version.
Looks like Shaolin Monks released on Germany has the fatality scenes censured too, but I can't confirm that.

Zelda Ocarina of Time when you meet Ganondorf in his tower, and kill him... he spews Red blood in US game and Green blood in PAL versions. Mystical Ninja Has different title screen logo on US version than PAL. StarFox 64 Games name got changed to Lylatwars 64

Tekken 3 japanese ending of either Anna or Nina has been more "softcore" in the pal version. Soulblade(Souledge) has the name changed, and the numbchucks were edited to a "nonchuck"
i remember that the us release of gomens great adventure for 64 had all the music cues cut out, as well ganon spewing blood was actually aval in all or most gold cart versions of its us release, i can confirm this as i had one of the first gold carts, as well they cut out some muslim refrences as well as a muslim chant that was in the fire temple music.
Also the ending is cut as well !! I have a beta of the Konami release of NCS Misaya's amazing shooter which actually has a sound test and the full endind. Still missing the anime characters though and the guy shooting himself. Yakumo
The japanese version has new levels, more caharcters, loads of extra costumes with some being rather sexy oh and a slight tweek in the handling. Overall the JPN version is much nicer. Yakumo
