I got a fun one in GTA San Andreas (PS2). I was playing around with cheats, and i put a bomb close from a car, and when i exploded it, i was entering in the car... After that, the car was completely exploded and the car was still working! I caught a footage of how fucked up was that car...
I was playing SA2 on Dreamcast other day. Had many level glitches resulting in many lives lost. Pissed me off...
Running on original hardware, or Hackintosh? Also, I find Flash on all platforms has become really tetchy.
Pokemon. I don't remember which version because it happened on several. I always forgot to save until I was ether done playing for the day or something happened that caused the game to freeze up. ALWAYS SAVE
Uncharted 2 on the PS3, jumping or falling on certain item u may be stuck there until u kill yourself or reload the game. Uncharted 1 was good ~
The top two for me would be the save corruption on Zelda. I had gotten all the way to Gannon but died and saved my progress. The next day I booted up the game and my save refused to work. After a few weeks I finally restarted the game and finally beat Gannon. This was my first experience with battery failure on a game cart save. The second is my unfinished game of FF7. I was on the second disc and had saved for the day. A few days later I booted up the game and my memory card had been corrupted. Every game save including FF7 was wiped out. To this day I have never completed that game. I also learned never to buy third party extra capacity memory cards. I don't remember the maker or model but it had a small button you press to change memory space.
besides the events not triggering, dudes running in circles, going out to the infinite, one of the most bizarre glitches was the T-Rex falling from the sky after falling from a pit in "jurassic park episode 2: the chaos continues". can i put videos? i managed to catch this on peter "jackson's king kong", for the Gamecube this glitch is insane.
A mission on GTA: VC Stories on the PSP. I attempted a mission to kill some bikers (I think) several times before finally nearly finishing the mission and needing to kill one last biker. When I found him, he was has stuck against a building. I couldn't kill him at all. I was so angry I never played the game again.
This happened to me when Mortal Kombat 2 for megadrive 2 came out, i ended up getting it (cheers mum), I remember i was on the spiked roof level and i was reptile fighting Kung Lao. I was practicing reptiles fatality when right as the match was about to start the game went black and my tongue fired out and took off his head. Then i won, flawless victory, fatality! Then went onto round two like normal. Never heard or seen info about that glitch since. Strange... I musta spent $15 on the 0900 cheat code line ( i was a regular) excitedly squealing down the phone line like a 8 year old should.

I was playing parasite eve today, got the second fight with eve. Went to change my weapon and then afterwards my controller wouldn't respond and I got killed. That's what I get for not saving I guess
Some glitches I have encountered in Lord of the Rings Online. Travel mounts stopping in place. Invisible/untargetable enemy in Misty Mountains. Couldn't see, attack it. Fortunately, a friend did and was able to beat it. Moria: Travel Goat getting stuck, like horses in other areas. Travel Goat missing path(or something) and dumping me in a water pool. Still with the riding animation. Could have been worse had the fall been in magma/bottomless pit... Invisible/untargetable enemies. Two of them together. This time, it happened to us both, so KO. Real unpleasant one so far. Here is a funny one. It can be repeated as often as you like. Get a travel mount and pick any location you can get to. Select an enemy/npc and try throwing a hatchet, or use bow/crossbow/other skill. You will be dismounted. Now, go to a place with a door and enter it. You will be on the travel mount and it will be running and half embedded in a wall. Target an npc again, and try using skill. You will be dismounted again. Now go outside. You'll be on the mount again, and it will try going to the destination. It will likely hit a wall or object but will keep on running. I have yet to try this in Moria and return to a city to see if I would be on a travel Goat, or magically replaced by a horse. Edit: Don't remember anything as bad happening in Final Fantasy XI... Think there were some minor stuff, but nothing like Lord of the Rings Online. Edit 2: Have some screens somewhere, could look for them in the screens folder. Edit 3: Remember an FFXI glitch. In Giddeus http://wiki.ffxiclopedia.org/wiki/Giddeus, there is an odd plant with a strange habit of appearing/disappearing depending on where you view it, while the others are not effected. Totally game breaking.
One of the biggest glitches that I have ever experienced occurred in Super Mario 64. I was in the large room in Course 6: Hazy Maze Cave, where the giant boulders fall into the pit. I myself happened to fall in the pit, but Mario got stuck on one of the corners where the pit meets total darkness. He kept tumbling back and forth repeatedly against the two sides of the pit which met at that corner, and he simply would not fall into the black pit and die, so I had to reset the game in order to continue.
The bug that killed my confidence in Bioware(after getting bruised by Dragon Age II). In Mass Effect 3, if you imported a Mass Effect 2 save file where you had imported your Mass Effect 1 save file, your characters appearance would not transfer. Now my created character, that I had been using across 2 games and nearly 100 hours, was effectively dead. My created character WAS Shepard. I could try and recreate him, but some of the options were no longer avaliable, so he wouldnt look the same. How in the world did the game ship with this bug? They just screwed over their most loyal fans. So I decided to hold off until this was patched. Of course the controversy over the ending of Mass Effect 3 was everywhere and I was trying to dodge spoilers left and right. After 2 weeks I gave up hope and just went ahead and played the game with a doppelganger. 2 weeks after that they patched the game so your appearence would transfer correctly, but the damage was done.
I played through Halo 2 on Legendary with the skulls active. After a couple of hours fighting the boss I won. Everybody was superhappy and hoped there would be an alternative ending. However, the game did show me the normal ending (not even the legendary ending). I was super-angry after that. I considered it a bug at the time, but it might be normal that the cut-scene is the normal one when collecting skulls.
I have a SNES Mario Allstars cart that doesn't let me pass World 8 level 2 (actually might be 3) on the Lost Levels. SNES just freezes. Really frustrating as it's a bit hard, and I've still not completed it. One that put me off playing a game was US Metroid Prime on Gamecube. I reached a point with a lift and I think an animation or cut scene is played, and played, and played. It happened twice to me, then I gave up on the game and never played it again..
I have an SNES Mario All-Stars cart with a very similar problem. If I beat World 2 in Super Mario Bros. 3, the game freezes, so if I want to see every level in the game, I have to use a whistle to skip World 2's boss level.
Fallout 3 and New Vegas constant lockups on PS3. Which to this day is still a known issue with no fix.
