It wasn't me playing, but once i witnessed some weird bug happening. Me and two friends were playing Battletoads, when one of them got to the turbo tunnel stage (which he was never able to get through) and suddenly, the toad started flickering and passing through the barriers like it was invulnerable! We immediatelly got pretty excited, but the little brother of the guy who was playing just came and... Pushed the reset button. I really wanted to know what would have happened if that brat hadnt done it.
rachet & clank crack in time ps3......fighting valkarie space battle. its a known issue where if you die when the valkarie ship blows up you just end up flying in the battel area with no one else to fight. xenoblade 3ds apparently theres this glitch that freezes the game in combat while having a vision, you can move the camera and move the ability selector but nothing else anyways tldr grind for 3+ hours and end up at a crucial boss fight, game freezes and that thing happens. the word "fuck" is said a concerning amount of times. 4 reddit threads and like 20 minutes of dicking around but alas, nothing gets done at all. like 3 hours wasted. suicidal thoughts ensue. the usual

WWF No Mercy is up there. That saved data loss keeps going and going. You're filled with dread any moment all the game's champions, shop items, created wrestlers, everything, will *poof* AGAIN. In the middle of story mode, created wrestlers revert to default nobodies.
For those people playing Oblivion/Fallout 3/Fallout New Vegas/Skyrim (and probably Fallout 4 now that the Fallout 4 mod tools are available) on PC, there are "unofficial patches" for those games (and the DLC) that aim to fix as many of the glitches and bugs as possible. Well worth it if you are playing the games (especially Oblivion/Fallout 3/New Vegas) on PC.
Playing The Crew on the Xbox One these days so centre it was free for Gold members. Of course being a Ubisoft game it's always bloody crashing and hanging
Playing GTA V or MGS V on PS4 and crashing at the most random times. Even after sending reports, I kept on getting them.
When I was really young I would play Bugs Bunny Birthday Blowout, because I was an avid looney tunes fan. After dying so many times, which seemed to take forever in the game, you would be presented with a try again screen which would look like this: However, the graphics glitched and instead bugs bunny did not have any pupils. This completely traumatized me. There was also one time me and my brother were playing Donkey Kong Country, Head to Head mode. We made it all the way to the second mine cart stage and something happened during the stage to corrupt the graphics and eventually freeze the game. My brother pushed reset and the frozen image was still there, but with more corruption. After a few more resets, he turned it off and on again. Our save data had been erased. Completely killed our co-op session and we didn't pick it up again. There are also a few funny bugs put in developers because they couldn't finish the game or some other stupid reason. I have 3 of them. 2 of them the same game. 2004 I got Skies of Arcadia for Gamecube. I was playing for 55 hours but I got a corrupt save file. So I decided to play again and this tgime 50 hours in I saved but power went out and yes lost my data. Not until June of this year on Dreamcast did I finish the game. Still have my GCN copy tho. Other one is few days ago. Playing KOF14 and I was dominating a match. Something I rarely able to do. Then my controller stopped working. Not a dead battery just would not sync. I lost the match. Then PSN went down. I can't tell you how fucking pissed off I was. I never scream and yell playing games but I was this time. Not even the save data thing made me this angry. Tho it was close.
The worst one I can remember has to be Ultima 9. That game was buggy as hell and constantly crashed to begin with but there was this one time when I finally got to a town called Moonglow. It was this city with these weird buildings that were build on top of each other. Anyway, I got into town and started to explore, when suddenly the framerate dropped to like 1-2 fps. It was a very resource heavy game at the time and very badly optimized so framerate drops weren't anything new, except this one was the worst yet. Then I noticed that one of the buildings started to follow me. No matter what I did, the building would always remain a set distance away. After a while, the game blue screened my windows and corrupted my saves. Never touched it again.
Metroid Prime on the Gamecube, crashed on me in one of the lifts just after I'd fought the big boss monster made of rocks (Thardus...?) before I'd had a chance to save. It had been a pretty challenging battle and I was so pissed off when it crashed I nearly smashed my Gamecube to pieces there and then. Luckily I was able to control myself. Also, not really a bug as such, but I had a third party Gamecube memory card that became corrupted and I lost my F-Zero GX save file that had all the AX tracks unlocked on it. I could've wept. Instead, I sat down and spent the afternoon going through the absolute torture of re-unlocking them (using a little Action Replay cheat-ification for infinite boost. Still bloody difficult though, even with it).

First time playing GTA:VC was a used copy from an early childhood friend. He gave me his copy. Lest to my knowledge, his younger brother (at the time 4) had his grubby, snot covered hands all over it. When i received the copy, it looked like it went through a surfacer multiple times and was covered with a thick crusty surface. This disk was gross and obviously beyond well worn. To my surprise, it worked reliably. If you played enough times, you could tell how long you could play before the game crashed, where to travel (because you'd otherwise fall through the streets and would keep falling until the game crashed) and just generally how to play the game without crashing. Unfortunately, playing the game long enough to figure out the work arounds to playing the game was the frustrating bug (or bugs). You could only play the game 5-15 minutes until you had to save (unless you didn't want to progress in the game), you couldn't purchase weapons, you couldn't do most optional money making side missions (Police missions, firefighter missions etc.) and some part of the roads you had to drive on what seemed to be a tightrope of a lane. You couldn't collide with other cars and had a small margin for error. It made a 3 day game almost 2 months. The last few missions (especially the last racing one) almost turned me off from the GTA franchise forever. It was one of the worse summers I had as a kid.

I'm playing Silent Bomber (neat game) and reach a cutscene where the main character rescues his commanding officer from prison. As the two characters stand there talking, the game glitches out, the English voices stop, and random Japanese voice tracks start playing. The oddest part? The game didn't have any player options to use the Japanese voices. I guess they were still in there someplace. It's not the most frustrating bug I've encountered, but it's the strangest.
The DOS game "Striker" : My version probably was infected with a "paritiy B virus"...not sure... the game crashed randomly, with a loud PC beeper noise and frozen graphics, crashing back to DOS in VGA mode. Very scary. But I loved the game ! Only solution was to turn off the 8086 Sanyo PC with 8 MHz.
